          LETTER FROM THE OWNER TO EVERYONE

                                          Hello Everyone!
My name is Simon Akpa and I am the owner of Arizona Human Services, AHS. I would like to share with you some information about myself and how the company began. 

Simon Akpa started working in the Human Services Industry in 1990 with the Arizona Attorney General’s Office, Human Services Division; Community Relations Section as the Education & Outreach Coordinator, in the areas of Arizona Landlord & Tenants Rights, Immigration related employment designed to help Arizona employers and the community.

From 2006 – 2011, Mr. Akpa was a faculty member with the University of Phoenix where he taught two (2) Human Service courses (HS/205 & BSHS/312;  Human Services Process & Self Care, as well as  HS/215 & BSHS/322; Introduction to the Human Services Profession. Additionally, Mr. Akpa worked for a large group home and behavioral health company as a training coordinator/administrator. In February 2020, Mr. Akpa began working with another social and behavioral health company known as Arizona’s Children Association. He worked with this company as a Clinician/Therapist until May 2020 when the Pandemic outbreak, COVID-19 made him resign and focus on Telehealth services etc.

Mr. Akpa’s overall responsibility within each of these positions was to ensure residents, members, and students received the highest quality of care and education. This included active participation in quality improvement initiatives, review of clinical records to identify areas of weakness, member’s/client counseling, and corroboration with Administrator/CEO to institute policies and procedures for resident, member, and student member care.

His love for the field and working with this population inspired him to open his own business. In February of 2020 he opened a Telehealth company, NEMT,  Respite/Habilitation service company as a contractor with the State of Arizona; DES/DDD. 

Personally, I have been blessed with the experiences I had and my business and I enjoy giving back to the special needs population and families that I served for more than 20 years.

PHILOSOPHY AND VISION TO ACCOMPLISH THIS MISSION

We offer Respite, NEMT,  and Habilitation Supports

AHS will respect the dignity and well being of all individuals. Promote a positive working environment for all staff. Provide quality services in a caring, non-judgmental manner. We accept ages 8 and up with the primary diagnosis of DD. Our staff will be up-to-date on current trainings relevant to Respite Care and all mandated trainings from the State of Arizona, DD Services.

SERVICES COMMUNITY LIVING AND SUPPORT


Community Living and Support is an individualized service that enables the waiver beneficiary to live the home of his/her family or successfully in his/her own home, the home of his/her family or natural supports and be an active member of his/her community. A paraprofessional assists the person to learn new skills and /or supports the person in activities that are individualized and aligned with the person’s preferences. The intended outcome of the service is to increase or maintain the person’s life skills or provide the supervision needed to empower the person to live in the home of his/her family or natural supports, maximize his or her self-sufficiency, increase self-determination and enhance the person’s opportunity to have full membership in his /her community. Community living and support enables the person to learn new skills, practice and/or improve existing skills. Areas of skill acquisition are: interpersonal, independent living, community living, self- care, and self-determination.

COMMUNITY NETWORKING SERVICE 

Community Networking services provide individualized day activities that support the participant’s definition of a meaningful day in an integrated community… setting, with person who are trained. This service is provided separate and apart from the participant’s private residence, other residential living arrangement, and/ or the home of a service provider. These services do not take place in licensed facilities and are intended to offer the participant the opportunity to develop meaningful community relationships with non-challenged individuals. Service are designed to promote maximum participation in community life while developing natural supports within integrated settings. Community networking services enable the participant to increase or maintain their capacity for independence and develop social roles valued by non-disabled members of the community. As participants gain skills and increase community connections, service hours should fade; however a formal fading plan is not required.

Community Networking Services Consist Of:
Participation in adult and children education, Development of community based time management skills. Community based classes for the development of hobbies. Leisure/cultural interests; Volunteer work. Participation formal/informal associations and/or community groups, Training and education in self-determination and self-advocacy, Training in using public transportation, Training and participation in health and mental health education, Inclusion in a broad range of community settings that allow the participant to make community connections; and/or events, and to have planned time for him/her and/or family members. Respite may include in and out-of-home services, inclusive of overnight, weekend care, emergency care (family emergency based, not to include out of home crisis) or continuous care. The primary caregiver is the person principally responsible for the care and supervision of the participant or individual. Our organization always appreciates the generosity and involvement of people like you, with every effort going towards making Arizona Human Services – AHS an even better Social Services Organization than it already is. We want to provide you with the correct and appropriate information pertaining to your mode of support, so don’t hesitate to contact us with your questions. 

RESIDENTIAL SUPPORTS  

AHS Residential supports consist of an integrated array of individually designed training activities, assistance and supervision. Residential supports include: 1. Habilitation services aimed at assisting the participant to acquire, improve, and retain skills in self-help, general household management, meal preparation, personal finance management, socialization and other adaptive areas. 2. Training outcomes focuses on allowing the participant to improve his/her ability to reside as independently as possible in the community. 3. Assistance in activities of daily living when the participant is dependent on others to ensure health and safety. 4. Habilitation services that allow the participant to participate in home life or community activities. 5. Transportation to and from the residence and points of travel in the community is included to the degree that they are not reimbursed by another funding source.  

RESPITE INDIVIDUAL SERVICES

Respite services provided periodic support and relief to the primary caregiver(s) from the responsibility and stress of caring for the participant. This service enables the primary caregiver to meet or participate in the planned: 1. Emergency Events 2. Planned Time for him/her 3. Planned Time for the family members. Respite may include in and out-of-home services, inclusive of overnight, weekend care, emergency care (family emergency based, not to include out of home crisis). The primary caregiver is the person principally responsible for the care and supervision of the participant and must maintain his/her primary residence at the same address as the participant.

Non-Emergency Medical Transportation (NEMT) Services

 Arizona Human Services LLC provides  Respite & Transportation care relief for primary caregivers. We can arrange for just an afternoon or for several days or weeks. We provide care at home, in a health care facility, or at an adult and child daycare center. 

AHS NEMT Services help people, AHCCCS members get to pre-scheduled health care appointments, including doctor visits, rehab facility, clinical testing, school, follow-up exams, and more.
